Terms & Policies

 

Payment:

Payment for all services is due at time of service. Payment methods accepted are cash, check, or credit card. Please note that a 10% fee will be charged for any returned payments by check. It is required that all clients have a credit card on record with us, regardless of preferred payment method. This will only be charged in the event of returned payment or failure to cancel a scheduled appointment.

CANCELLATION POLICY:

It is your responsibility to notify your therapist at least 2 hours prior to a scheduled appointment if you plan to cancel. Failure to do so will result in a charge for 50% of the full session fee. Please contact us as soon as possible if you fail to show for a scheduled appointment due to unforeseen circumstances. Unforeseen circumstances and accidents will be handled on a case-by-case basis.

Insurance:

Avidus Therapy, LLC is a private-pay business. We do not bill insurance, and all payment for services is due at time of service. If you have financial need, and cannot afford full service fees, please contact us prior to your appointment. Limited sliding scale slots may be available for those in financial need of them.

Confidentiality POLICy:

All information discussed within sessions, and shared with Avidus Therapy, LLC in general, shall remain private and confidential unless otherwise required by law, or if legal actions are taken. Limitations to confidentiality include, disclosure of imminent harm to yourself or another person, ongoing or active abuse of a child or elder, or subpoena by a court order. A patient waives their right to confidentiality under our policies if they choose to pursue legal action against Avidus Therapy, LLC or its therapists.
